Dr. Damion thanks his patients for their confidence as he announces his retirement from his ear, nose and throat practice. His office will close on December 6, 2019.
His patients are entitled to a copy or transfer of their medical records. He will be custodian of these records.
If you would like copies of your medical record to be sent to you or to your new ear, nose and throat physician:
- Please fill out the attached authorization form and return it to Dr. Damion:

- Any name changes
